Use this form to pay in US$ by check
- After download, choose "open" from the File Download dialog box, fill out and print the form and mail it to the ISCH Treasurer, Dr. Tamas Alexy (mailing address is on Page 2 the form).
- Please include a US dollar check made out to ISCH. The "Professional Information" section on Page 2 is optional for existing members but is required for new members.
- ISCH cannot accept checks in currencies other than US$ due to high bank charges.
- If you select a journal subscription (Page 2), your request and your contact information will be forwarded to the publisher, IOS Press. Do NOT send payment for Journal subscriptions to ISCH. IOS Press will contact you directly.
Use this form to pay in EURO by bank transfer
- Refer to the account details on Page 1 of the form and send your membership dues to ISCH via bank transfer. Then fill out and print the form, and mail it to Dr. Max Hardeman, the ISCH Secretary (mailing address is on Page 2 the form).
- Include a printed receipt or some other proof of payment along with the form. The "Professional Information" section on Page 2 is optional for existing members but is required for new members.
- If you select a journal subscription (Page 2), your request and your contact information will be forwarded to the publisher, IOS Press. Do NOT send payment for Journal subscriptions to ISCH. IOS Press will contact you directly.
